HGV (D4) Medicals
The D4 medical is an examination that consists of an eyesight test using a Snellen chart, blood pressure checks to ensure your BP meets the given criteria, and a questionnaire regarding your medical history.

These medicals are performed by fully Qualified Doctors and provides a comprehensive assessment with completion of the D4 form. Calluna Medical will require a summary from your GP in order to ensure that the form and your assessment is completed to the standard required for the DVLA.
